Your Rights

You have the right to:

  • Request corrections — Ask us to correct inaccurate or incomplete information.
  • Confidential communication — Request that we contact you in a specific way (for example, by secure email or phone).
  • Limit sharing — Ask us not to share certain information.
  • Accounting of disclosures — Request a list of when and with whom we’ve shared your information.

  • File a complaint — If you believe your privacy rights have been violated, you may file a complaint with Marula Rehab Telehealth or with the U.S. Department of Health and Human Services.

  • Bill for services — Submit claims and verify coverage.

  • Conduct research — When permitted by law and approved by oversight boards.

  • Respond to organ and tissue donation requests
  • Work with medical examiners or funeral directors
  • Address workers’ compensation, law enforcement, and other government requests
  • Respond to lawsuits and legal actions
  • We will not use or disclose your information for any other purpose without your written consent.

 

Special Protections for Substance Use Disorder Information

  • Records related to substance use disorder treatment are protected under 42 CFR Part 2. We cannot disclose any information identifying you as receiving substance use disorder services unless:
  • You provide written consent;
  • A court issues an order meeting specific legal requirements; or
  • Disclosure is permitted under federal regulations for medical emergencies, audits, or research.
  • These protections apply to all Marula Rehab Telehealth services, including assessments, counseling, and referrals. Your records cannot be used in civil, criminal, administrative, or legislative proceedings without your consent or a court order.

 

Telehealth‑Specific Privacy Practices

  • Because Marula Rehab Telehealth operates exclusively through secure virtual platforms:
  • All sessions are conducted using encrypted, HIPAA‑compliant video and messaging systems.
  • Electronic records are stored in secure, access‑controlled systems.
  • Staff are trained in remote confidentiality procedures and Utah DHHS telehealth standards.
  • No in‑person services are provided, and no physical files are maintained.
  • You may request copies or corrections electronically through our secure patient portal.

 

Our Responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.
  • We will notify you promptly if a breach occurs that may compromise your information.
  • We will not use or share your information other than as described here unless you give written permission.
  • You may revoke your authorization at any time.
  • We will provide language assistance and accessibility accommodations upon request.
